[6bone] Question about register 6bone (ask again)

In the web page http://eng.hexago.com/cgi-bin/registry/creform.pl, there're 
some fields. When start a new registry, I creat a person object, what fields 
must be fill up except "mnt-by"?  I always show some errors.  Who can help 
me?
