Found the answer.  For anyone else that needs to do this:


Insert your bootable floppy and get a root shell open

 # mkdir junk
 # cd junk
 # dd bs=2x80x18b if=/dev/fd0 of=boot.img
 # mkisofs -b boot.img -o boot.iso .

Burn a CD using the boot.iso file as source.
