Whenever I open a new account, I state the following:

"This conversation is being recorded for legal reasons. I would like to be
removed from ALL distribution lists you currently maintain. I also don't
want any of my information shared with 3rd parties or internal affiliates.
In addition, I would like to be removed from all marketing campaigns, and
I don't want to receive blank checks from your organization. To summarize,
I would like the following to occur:

1) I do not want to receive marketing literature (including blank checks)

2) I don't want any of my information shared with third parties or
   internal affiliates

3) I don't want my information sold to any third parties or added to any
   distribution lists

4) I would like a copy of your privacy statement

Did you capture each item I listed above? When will each item be
completed, with dates?"

This has worked pretty well so far :)
