I haven't found a book which covers everything.  But if I was choosing
a first book on the subject, try "IEEE Recommended Practice for Powering
and Grounding Sensitive Electronic Equipment."  Also known as the
IEEE Emerald Book (IEEE 1100-1992).  Perhaps the most valuble part of
the book is the extensive list of references and bibliography.
